Reports to: Program Manager

Summary Provides supportive case management, financial management and life skills training for clients in program.

Essential Duties and Responsibilities: include the following (and other duties as may be assigned)

Provide ongoing case management to clients residing in Emergency Housing Voucher Program

Assist clients in housing navigation and location

Document all client interaction and maintain accurate paperwork/ reports

Communicate effectively in oral and written forms with clients, their families, coworkers, supervisors, and other service agencies and the community

Ability to hold clients with clinical needs in positive regard and provide services without judgment

Provide mediation and advocacy for participant between the landlord and surrounding community members

Secures information such as medical, psychological and social factors contributing to the client’s situation and evaluates these and client’s capacities.

In partnership with the client, creates individualized service plans and performs follow-up to determine quantity and quality of service provided to client and status of client’s case.

Meet with clients to provide strengths-based case management and service coordination such as coaching around independent living skills, employment assistance and linkage to community providers for services

Attend training on best practice models that apply to the population served (such as Motivational Interviewing, Strength Based Case Management, and Trauma Informed Care)\

Teach clients principals of being a good tenant and life skills

Provide other supportive services needed for the client in order to help them maintain housing

Qualifications:

Bachelor’s degree in social work or related field required

Two year minimum experience working in homeless services, mental health, substance use/co-occurring disorder or with vulnerable population providing intensive case management services.

Ability to work independently and within a team environment and exercise mature judgment

Must have good computer skills and strong written and verbal communication skills

Pass a background check, pre-employment drug screening and must have a valid driver’s license with a clean driving record

CPR/First Aid certification (on-site training) and TB Testing

While performing the duties of this job, the employee is occasionally required to sit and talk or hear. The employee must occasionally lift and/or move up to 25 pounds.

Job Type: Full-time

Salary: $26.00 – $28.00 per hour

  • 8 hour shift

Experience:

  • Case management: 1 year (Preferred)
  • Public Housing Specialists: 1 year (Preferred)
