This position is eligible for a $3,500 Sign-on Bonus for candidates with at least one year RN experience.

 

Cottage Center for Population health works to improve health and wellness – focusing on equity for the most vulnerable – in our hospitals and our community. Our team works in partnership with our community members, partners, medical and service providers, and hospitals.

This position is based in the community, working closely with partners and staff to support vulnerable populations.

Provides basic medical care, health education, screenings, and health outreach for vulnerable community members as part of a collaborative community programming serving local community sites and neighborhoods, including, but not limited to, working with and alongside, people experiencing homelessness, community health workers, faith communities, and community members with high social and behavioral health needs. Serves as the clinical liaison with Santa Barbara Cottage Hospital and community partners while working in a community.

California Registered Nurse license

Valid class “C” California Driver License. Verification of acceptable driving record from the California Department of Motor Vehicles.

Knowledge and understanding of the impact of social determinants of health on a person’s health and access.  Knowledge of advanced clinical standards, practices, and procedures for managing high-risk/at-risk patient populations.

Bilingual (Spanish-English) and bicultural provider with lived experience in and with target community. Previous experience working with community health workers or promoters and/or faith communities is preferred.

One (1) year direct patient care experience in a clinical setting or managing high-risk patient populations required.
