FLSA Status: Full time, Non-Exempt, Hourly (DOE) Schedule: Monday – Friday 8 am – 4:30 pm

Compensation: $27 – $29 per hour, commensurate with experience. Holiday, vacation, and sick pay. Health Insurance stipend, Company Vehicle.

The mission of Doctors Without Walls-Santa Barbara Street Medicine (DWW) is to provide free, volunteer medical care for the most vulnerable, underserved populations in Santa Barbara County, when and where they are in need, including in times of disaster and to provide education and training for DWW volunteers and others, in order to promote the excellent practice of humanitarian medicine in our own community.

Position Objective: The ECM Case Manager supports homeless individuals and families residing in Santa Barbara County. The position is case management “on-wheels” and liaison to community outreach partners.

Essential Job Functions & Duties:

An Enhanced Care Management (ECM) Case Manager provides intensive, person-centered, and community-based care coordination for individuals with complex medical, behavioral health, and social needs. This role extends beyond traditional case management by meeting clients where they are—including on the street, in their homes, shelters, hospitals, clinics, or other community settings—to build trusting relationships, reduce barriers to care, and improve health outcomes and housing stability.

The ECM Care Manager is responsible for conducting comprehensive client intakes, completing biopsychosocial and health risk assessments, developing individualized care plans, and coordinating services across medical, behavioral health, substance use treatment, housing, and social service systems. Care Managers work collaboratively with multidisciplinary teams, health plans, community partners, and service providers to ensure clients receive timely, coordinated, and culturally responsive care.

Key responsibilities include ongoing care coordination, appointment scheduling and accompaniment as needed, transportation coordination, benefits navigation, referrals to community resources, crisis intervention, advocacy, documentation in electronic health records, and monitoring client progress toward established goals. ECM Care Managers maintain regular client contact through field-based outreach and engagement, adjusting care plans as clients’ needs evolve.

Success in this role requires strong communication, critical thinking, trauma-informed care, motivational interviewing, and problem-solving skills, as well as the ability to work independently in community settings while maintaining accurate documentation and compliance with Medi-Cal ECM requirements, organizational policies, and applicable regulations. Meet and assist clients daily, via phone or in person, with medical care, substance abuse treatment, mental health counseling, and housing opportunities. Document all contacts and outcomes in the Electronic Medical Record, uploading necessary information to an external database on a case-by-case basis.

Adhere to all local, state, and federal guidelines and laws I.e. HIPAA, OSHA, FLSA. Maintain and develop relationships with federal, state, and local agencies that serve as referral resources for clients. Perform special assignments to this unique function (homeless counts, data processing, presentations). Other duties, as relevant and assigned by Program Managers and Executive Director.

Skills & Abilities:

● Adept in MS Office (Excel, Word, and Google).

● Skilled at working with diverse populations and engaging clients. Ability to communicate professionally and with integrity.

● Ability to work independently or as a member of a team.

Excellent problem-solving skills.

Knowledge:

● High School Diploma or GED (AA or BA preferred)

● Bilingual

● Valid Driver’s License + Driving record required

● Experience with the homeless population.

● Experience in street outreach and/or the homeless shelter environment.

● Knowledge of local resources for the homeless and underserved populations.

● Must be able to lift 40 pounds.

● Work requires the following motions: bending, twisting, squatting and reaching.
