Job description
Enhanced Care Management is a whole-person, interdisciplinary approach to care that addresses the clinical and non-clinical needs of high cost and/or high-need Medi-Cal recipients through services and comprehensive care that is community-based, interdisciplinary and person-centered.
The Practice Manager, ECM is responsible for directing and managing the daily operation of the Integrated Care Clinic.
The Practice Manager, ECM reports directly to Program Director, ECM.
Essential Duties and Responsibilities
- Determines Clinics priority needs based on current patient volume demands for service
- Directs staff to accomplish objectives and goals in accordance with Clinics mission and vision in excellent patient service and growth
- Develops business with organizational resources such as physicians, medical groups, healthcare facilities, agencies, and other community sources when appropriate
- Participates in the development and enforcement of departmental policies and procedures
- Prepares and maintains statistical reports of the Clinics
- Conducts periodic performance evaluations of ECM staff
- Utilizes leadership skills and technical knowledge required to manager the daily operation of the clinic
- Consistently serves as a role model in providing outstanding service to patients, physicians, fellow employees, and guests. Maintains professional level of conduct by demonstrating courteous and positive behaviors
- Effectively utilizes communication and interpersonal skills, such as problem-solving, negotiation, and conflict management, to maintain productivity and a good working environment
- Utilizes department equipment/supplies with efficiency and cost-effectiveness, while also managing vendor relations for specialty equipment/supply needs
- Displays initiative to see processes through to completion
- Consistently responds to requests in a timely manner

- Experience of working with a variety of different stakeholders, including residents, local groups, and organizations across the voluntary and public sectors
- Understand, maintain, and uphold the mission and vision of Sanctuary Centers of Santa Barbara
Education and Experience
- Master’s Degree required
- LCSW in good standing in California and licensed for at least two years required
- Extensive experience working in community outreach and engagement, particularly with disadvantaged groups
- Strong written and verbal communication skills required
- Strong computer skills are required especially with case management data systems
- Must be hard working and reliable professional willing to be a role model
- Bi-Lingual, English and Spanish
- Must possess a valid driver’s license and motor vehicle insurance to operate a motor vehicle in the State of California
- Must pass a criminal background check
Job Type: Full-time
Pay: $85,000 – $100,000 per year, DOE
