TITLE: Life Skills Coach
Immediate Supervisor: Independent/Supported Living Coordinator
Supervision Exercised: Persons Served
SUMMARY:
Under the supervision of the Program Director and Coordinator, this position is
responsible for the implementation of Individual Service Plans that will lead to or
continue to promote independence and personal skills and assist in providing a
safe and secure home environment for persons served in our
Independent/Supported Living program.
ESSENTIAL FUNCTIONS:
• Provide individualized coaching in daily living skills
• Provide education and support in the use of community resources
• Maintain accurate and current records in ID notes and communication logs
regarding information relevant to Individual Service Plans
• Support and assist persons served in following their daily activity schedule
including household meal preparation, maintaining sanitary and orderly
conditions in their home, other household tasks, budgeting, ledgers and
transportation as necessary
• Prompt and assist persons served in medication management
• Teach adaptive living skills and monitor the extent to which they are followed
• Participate in in-service training and staff meetings to facilitate professional
development
• Assist participants in all aspects of personal care, including medical/dental
appointments
• Communicates concerns related to program participant needs to supervisor in
a timely manner
• Assists individuals in attending social activities to further social skills
• Train program participants to access public transportation and assist persons
served in accessing benefits such as Social Security, SSI and Sec 8 housing
• Assist persons served in weight bearing and non weight bearing transfers
• Use crisis intervention techniques in emergency situations when persons
served are in danger of injuring themselves or others
• Complies with all Momentum WORK, Inc. and OSHA policies, procedures
and regulations
• Must be able to transport participants, unless waived by the Executive
Director.
• Other related duties, responsibilities, and special projects on an as needed basis
PHYSICAL REQUIREMENTS
• Ability to stand, sit for long periods of time
• Push, pull at times
• Ability to go up and down stairs
• Ability to speak, read, hear, and understand technical written material
• Lifting may be required; may exceed 25 lbs.
• Ability to bend, lift and carry
• Full awareness of environmental stimuli
• Ability to implement Crisis Management, Safety & Pro Act techniques
QUALIFICATIONS:
• High school diploma or equivalent required; Associate of Arts degree or
equivalent in related field preferred
• Experience working with persons with developmental and/or other disabilities
preferred
• Demonstrated ability to maintain confidentiality regarding persons served and
other related information
• Excellent verbal and written communication skills
• Ability to complete tasks with numerous interruptions
• Ability to work with little direct supervision
• Ability to manipulate numbers
• Compassion and respect for persons with disabilities
• May need to be available during any disasters, per disaster plan
• Must have reliable phone
• Must have personal car and valid insurance
• Valid CA Driver’s License
• Valid certification in CPR/1st Aid (Training available)
Note: At the discretion of the Executive Director, additional related experience and/or education may be substituted or waived in lieu of the requirements specified above under “Education and Experience.”
