PRIMARY RESPONSIBILITES
Under the direction of the Program Director, the Food and Emergency Services/Family Service Coordinator is responsible for coordinating the Food Program, providing utility and other types of assistance, providing case management and emergency services .
EXAMPLES OF DUTIES
· Administer, monitor and supervise all phases of the Food Program, including but not limited to the following; organize and implement weekly program, recruit, organize and maintain a base of volunteers, organize transportation and storage of food, process Food Bank applications, organize the donations and distribution of local fresh foods, order Food Bank products, complete all appropriate reports in a timely manner and assist with advertising the program to the community.
· Coordinate the Fresh Rescue program.
· Interview clients and assess their situations, capabilities and problems, to determine what services are required to meet their needs.
· Case manage clients whose needs require more than one PHP service and/or referral to programs/services of outside agencies; coordinate services and follow-up on appointments to create self-sufficiency.
· Update and maintain PHP Emergency Services and Food Program database.
· Maintain complete case files including the supplementary information needed to assist clients.
· Attend pertinent meetings with other community providers to ensure collaboration and coordination of Food Program and Emergency Services.
· Enroll clients in mainstream benefits such as MediCal and CalFresh.
· Establish and maintain close liaison and working relationships with personnel in shelters, law enforcement, schools, health care providers, public agencies and community-based organizations.
· Plan, organize and implement the annual Fulfill-A-Wish Program.
· Serve as the local “Toys for Tots” Coordinator.
· Coordinate and maintain a base of volunteers to work with all PHP programs.
· Prepare reports in a timely manner.
· Perform other related duties as assigned .
BA preferred, AA in Human Services or related field with 2 years’ experience or High School diploma with 4 years of experience. Bilingual English/Spanish required. Valid California Driver’s License and clean driving record. Preferred ability to operate 35′ box truck. Must pass the Department of Justice fingerprinting process. TB clearance upon hire.
