Position Overview
The Case Manager will be responsible for our family support programs in North Santa Barbara County. The Case Manager will work with the Senior Case Manager and Executive Director to ensure Hearts Aligned programs are being administered properly. The Case Manager will work with medical referral sources and ensure all families have completed all required forms; are assessed for eligibility; interviewed; and have a well-documented file to be accepted into one or more of Hearts Aligned programs.
Key Responsibilities
Client-Centered Support:
- Build trusting relationships with families, social workers, healthcare providers and partnering organizations
- Assess and address the unique needs of each critically ill child and their family
- Ensure services are delivered with empathy and cultural sensitivity
- Understand preferred communication style of families – verbal, texts, in-person, etc.
- Maintain confidentiality and adhere to ethical standards
Financial Assistance
- Work with each healthcare professional referral source to obtain a recommendation for service
- Review all applications to ensure they are complete
- Contact parents/guardians and conduct an initial phone interview and review of the application
- Conduct in-person interviews with the family
- Keep Family Information file and database up to date/current
- Document all interactions with the family
- Make a report of the recommendations to approve or reject application to the Program Director
- Interact with Social Workers and Case workers/managers, as necessary
- Facilitate family grants – collect bills/invoices, W-9 forms and complete expense forms. Invoices will be processed by another individual.
- Follow-up with families to check on their welfare on a regular and ongoing basis
Family Support
- Coordinate program item deliveries
- Coordinate and provide referrals to other local community resources for families
- Act as a liaison between families and available resources (e.g., healthcare providers, other local community services, etc.)
- Continually update available resources as new ones are discovered
- Coordinate delivery of household items to families
Advocacy
- Act as a liaison between families and available resources (e.g., healthcare providers, other local community services, etc.)
- Work with families to coordinate transportation through other local agencies
- Assist families with understanding health insurance bills and negotiating payments for past due or current bills
- Provide case management services – evaluate needs, prepare plans/goals with families
Fundraising/Community Engagement
- Participate in fundraising activities
- Attend events to promote community awareness
- Promote Hearts Aligned as appropriate
- Collect stories for publication
Qualifications
- Bachelor’s Degree preferred
- Candidate should have excellent written and verbal communication skills in English and Spanish
- Case management experience of 3-5 years preferred
- Computer skills are required, including the use of Google Drive and Salesforce databases
- The candidate should have excellent organizational and time management skills. Experience working with families in need is required
- Must have reliable transportation
Employment will be out of the organization’s Santa Maria office.
Compensation
- $25-$30 per hour DOE, full-time in Santa Maria
- Benefits include accrued vacation (after 90 days employment), holiday pay, sick leave, healthcare benefits may be available

Location: Santa Barbara, CA with travel to Santa Maria, CA.
