The Position
The Case Manager/Clinical Case Manager provides case management along with service and resource coordination with clients that are low-income and may be in crisis or emergency situations. Clients served include adults, seniors, and families. Case management focuses include housing stabilization and retention, access and engagement with physical and mental healthcare providers, and supporting seniors aging in place. The Case Manager/Clinical Case Manager will work collaboratively with clients, community-based providers, , and public and private agencies to guide households in overcoming obstacles to self-sufficiency. Through advocacy, planning, coordinating, and linkage to community resources, the Case Manager/Clinical Case Manager will identify risks, promote strengths, and equip clients with the skills and knowledge needed to overcome sources of stress, strengthen resiliency, and health disparities. Services are provided in community settings, FSA offices, and in the homes of clients. This position reports to the Housing Authority Program Supervisor.

Sample Job Duties and Responsibilities
1. Provide case management services, often face-to-face.
2. Conduct assessments evaluating the current status of clients’ mental, emotional, and behavioral health and considering relevant cultural issues and concerns.
3. Enroll eligible clients in case management services to include CenCal Health CalAIM programs (Enhanced Care Management), among others. Administer case management services, to include completion of required assessments and documentation for specific services.
4. Provide advocacy and assistance for residents in obtaining public benefits.
5. Foster positive interagency relationships and provide referrals for any indicated outside resources.
6. Support residents in accessing substance abuse counseling/recovery and/or medical/mental health services and provide coordination of behavioral health and clinical services.
7. Provide and/or arrange crisis intervention services, mental health assessments and treatment planning. (Clinical Case Manager only.)
8. Collaborate with Santa Barbara County Department of Behavioral Wellness to ensure access to treatment for qualifying residents. (Clinical Case Manager only.)
Employment Requirements:
1. HA Case Manager: Bachelor’s degree required, with preference of major course work in social work, psychology or another closely related field.
HA Clinical Case Manager: This position requires a registered AMFT, APCC, or ASW.
2. Bilingual (English/Spanish) preferred.
3. Ability to communicate ideas clearly and concisely, both verbally and in writing, is required.
4. At least two (2) years of experience in social case work, eligibility determination, social service or community resource referral, or closely related work preferred.
5. Minimum of two (2) years working with diverse communities, low-income clients, the homeless and formerly homeless, seniors, and/or people with disabilities preferred.
6. Knowledge of general interviewing practices and/or techniques; knowledge of case management practices and techniques, including crisis prevention and intervention preferred.
7. Excellent interpersonal, analytical, computer, written, and verbal communication skills required.
8. Professional attitude, enjoyment of teamwork, and ability to maintain confidentiality a must.
9. Reliable transportation, valid CA driver license, and proof of insurance required to travel throughout Santa Barbara.
Classification, Hours and Pay Rate:
• This position is full-time (40 hours/week), non-exempt, with benefits, including: medical, dental, vision, life insurance, flexible spending accounts and 403(b) retirement plan. Generous vacation, holiday and sick leave also offered.
• The pay range for this position is $24.00-$26.00/hour for a BA-level applicant, or $28.00-$32.00/hour for a clinical applicant.
