The Case Manager will be responsible for our family support programs in North Santa Barbara County – these include financial assistance, personal support and patient advocacy. The Case Manager will work with the Program Manager to ensure Hearts Aligned programs are being administered. The Case Manager will work with medical referral sources and ensure all families have completed all required forms; are assessed for eligibility; interviewed; and have a well-documented file to be accepted into one or more of Hearts Aligned programs.
Financial Assistance
- Work with each medical professional referral source to obtain a recommendation for service.
- Review all applications to ensure they are complete.
- Contact parents/guardians and conduct initial phone interview and review of the application
- Conduct in-person interviews with the family
- Keep Family Information packet and database up to date
- Document all interactions with the family
- Make a report of the recommendations to approve or reject application to the Executive Director
- Send approval and/or rejection letters to every family that has applied
- Interact with Social workers as necessary
- Facilitate family grants – collect bills/invoices, W-9 forms and complete expense forms. Invoices will be processed by another individual.
- Follow-up with families to check on their welfare on a regular and ongoing basis
- Update client database
Personal Support
- Work with the Project Manager for program item deliveries
- Determine type of volunteer needed and schedule volunteers
- Provide assistance to those families in need of transportation, household needs, shopping to name a few examples
Patient Advocacy
- Coordinate other community resources for families – Foodbank, CalFresh, CenCal Health, etc.
- Continually update available resources as new ones are discovered
- Assist families with understanding health insurance bills and negotiating payments for past due or current bills
POSITION REQUIREMENTS:
Candidate should have excellent communication skills in English and Spanish – written and verbal. Computer skills are required, use of Google Drive and databases. The candidate should have excellent organizational and time management skills. Experience working with families in need is preferred. Must have reliable transportation. Must have reliable internet access/connection.

Travel all of Santa Barbara County in your own vehicle.
WORK ENVIRONMENT
The employee will work remotely but must live in North Santa Barbara County. The employee must have a regular schedule and have approved work hours, between 9 a.m. and 5 p.m. PT. Hearts Aligned will provide remote employees with a cell phone, laptop, printer and paper. Equipment supplied by Hearts Aligned is to be used for business purposes only. Hearts Aligned is not responsible for expenses associated with working at home, including heat, electricity, internet or phone service.
