Join us for a special 2-part workshop and complete your MyCare advance directive. Advanced care planning is a way for adults of all ages, in all stages of life to prepare for the future. Advance care planning involves:
- Making decisions now about the types of health care you would and would not want to receive if you become very sick or injured and cannot speak for yourself
- Choosing a trusted person to make health care decisions on your behalf if you were unable to do so yourself
- Talking with your doctors and loved ones about your wishes and healthcare goals
- Documenting these decisions in a legal document called an advance directive. In Santa Barbara County, we recommend using the MyCare Advance Directive.
Day 1 – Thursday, April 10, 1:00 – 2:30 – Overview of advance care planning, health care decisions, and MyCare
Day 2 – Thursday, April 17, 1:00 – 2:30 – One-on-one help from experts and free notarization (for advance directives only). If you are updating a current advance directive or only need notarization, you have the option of attending Day 2 only.
