Full-time, Hourly Non-Exempt. The Supportive Services for Veterans Families (SSVF) Shallow Subsidy Case Manager works collaboratively with a multi-disciplinary team that assists participants in navigating through complex resources and processes to identify/obtain/sustain housing. This will entail working with veterans who have successfully worked towards attaining greater independence in relation to the level of on-going case management services provided by the SSVF program. The SSVF Shallow Subsidy service provides a new, unique service type for SSVF grantees to use in supporting Veterans in obtaining and maintaining permanent housing. The SSVF Shallow Subsidy module will provide “Light Touch” case management services, which can include mediation between landlord-tenant issues and ensure successful tenancies, which would include, but not be limited to, the following activities:
- Conduct initial assessment to determine client’s specific needs
- Create a Housing stability plan with client, which includes documentation of client goals.
- Coordinate goal related services, and provide ongoing monitoring of the Housing stability plan
- Tracking and documenting participant progress.
- Establish and maintain relationships with landlords, property management companies.
- Conduct inspections of potential permanent housing units where program participants are residing.
- The Shallow Subsidy Case Manager will be responsible for recertification of eligibility of each continuing household on an annual basis.
- Provide “light touch” case management services which includes but are not limited to the following activities: providing appropriate resources to assist participants in achieving their case plan goals; making referrals for services and tracking and documenting participant progress.
- Will provide referrals to community resources to assist participants in accessing employment and training opportunities.
- Deliver HOME-BASED CASE MANAGEMENT services to all veterans transferring to the Shallow Subsidy module of the SSVF program.
- Provide crisis intervention as needed.
- Provide tenant education: tenant rights and responsibilities, housing discrimination and communication with landlords.
- Maintain a regular and open line of communication with landlords in-order to assess the landlords’ satisfaction with the programs and address any questions or concerns that landlords may have.
- Document and maintain up-to-date information on services provided to participants in the Homeless Management Information System (HMIS). Housing Stability Plan, Habitability Checklist, and Check requests
- Participate in weekly case management meetings with the agency Homeless Services Director, SSVF Program Manager, and SSVF program staff to identify potential veterans that could become an ideal candidate for the Shallow Subsidy module.
- Participate in a monthly case conference meeting for current participants in the Shallow Subsidy module to identify ALL Housing barriers that can place a veteran at risk of homelessness.
- Will maintain a caseload of 10-15 veteran households
- Respond to complaints from landlords & participants related to housing conditions & provide mediation as needed.
- Adopts an “whatever-it-takes” approach in assisting clients to retain housing and independent living.
- The SSVF Shallow Subsidy Case Manager will practice the Housing First model and will operate from a Harm Reduction, Conflict Resolution, and Trauma Informed Care best practice model.
- Transport/accompany if needed to appointments and self-help activities.
- Attend and complete trainings as assigned by Homeless Services Director or Program Manager
- Other duties as assigned.
Monday – Friday 9:00 AM – 5:00 PM, some evenings, weekends, and holidays may be required. Participation in an emergency on-call as needed.
Most of the direct service work is performed in the community (e.g., clients’ homes) and between the two SSVF offices located in North and South Santa Barbara County. Thus, the SSVF Shallow Subsidy Case Manager must have the ability to walk, stand, drive intermittent or sometimes continuous basis, the ability to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead; ability to operate computer, fax and telephone; and the ability to lift up to 25 lbs. Starting salary is $25.00+ per hour (D.O.E.) with medical and dental benefits provided, 403b contribution and matching, as well as generous paid time off. Position begins asap
- A minimum of 3 years of non-profit or related experience strongly preferred;
- Bachelor’s Degree in Business Administration, Real Estate Studies, Human Services or comparable combination of education/work related experience required;
- Experience in a social service setting with working knowledge of case management systems and planning techniques;
- Knowledge of housing resources, subsidy programs, and the Continuum of Care for homeless persons;
- Experience working with homeless individuals and families is strongly preferred;
- Experience working in property management or real estate, and/or with business development is strongly preferred;
- Must be proficient in the following computer applications–Microsoft Word and Excel. HMIS a plus;
- Must have own transportation to conduct job related travel; and
- Must successfully complete a background check and meet insurance carrier’s requirements for approval to drive.
